WHAT IS A CONTINUUM OF CARE?
Continuum of Care (CoC) is the collective networks, institutions, and organizations that provide housing and services to people who are at risk of or are experiencing homelessness.
Federally mandated goals of the CoC include:
Promote a community-wide commitment to the goal of ending homelessness.
Provide funding for efforts to quickly re-house individuals and families who are homeless, which minimizes the trauma and dislocation caused by homelessness.
Promote access to and effective use of mainstream programs.
Optimize self-sufficiency among individuals and families experiencing homelessness.
In compliance with HUD guidance and regulations for operating a Continuum of Care (24 CFR 578), the following Written Standards and Policies have been adopted by the Winston-Salem/ Forsyth County Continuum of Care.
The Continuum of Care (CoC) is responsible for coordinating and implementing a system to meet the needs of the population and subpopulations experiencing homelessness within a given geographic area, which for the NC-500 CoC includes Forsyth County.
Both the Emergency Solutions Grant (ESG) Program Interim Rule and the Continuum of Care (CoC) Program Interim Rule state that the CoC, in consultation with recipients and sub-recipients of Emergency Solutions Grants (ESG) Program funds within the geographic area, are responsible for:
Establishing and consistently following written standards for providing Continuum of Care assistance,
Establishing performance targets appropriate for population and program type, and
Monitoring recipient and sub-recipient performance. All programs that receive ESG, City of Winston-Salem or CoC funding are required to abide by these written standard
Agency program procedures should reflect the policies and procedures described in this document.
The CoC strongly encourages homeless programs that do not receive these funding sources to accept and utilize these written standards.
